Wow! Impressive! - But how to do Index from Word 2003?

Writing my first illustrated eBook. One click in MS-Word 2003, wait a couple of minutes and my document is BEAUTIFUL! I'm impressed! Looks better than i expect from PDF and the many large images are stunning! Anyway, what a great value!

But my problem: Being new to PDF, i don't know how to get my Word 2003 Index to be active in PDF? I looks good, but has no links. The Table of Contents looks great and does link just fine. Do i need to use one of those left panel PDF type indexes? And how?
